    (Dis)honorable mention:

    Joe Son's nuts get punished. There were no rules in place against groin strikes at the time. Son is currently in prison for rape and torture, so in hindsight it's a shame Keith Hackney didn't punch them right off.

    Some back story for non-MMA fans...

    Wes Sims had a history of getting into bad blood feuds with all his opponents. Mir was no exception. He was disqualified in the first fight for head stomps in conjunction with using the fence for leverage. They rematched in 2004 and Mir knocked him out. Sims lost his next UFC fight (also controversially after being bitten on the chest) and has toiled on the indy circuit ever since.

    Paul Daley was immensely frustrated with Josh Koscheck during their fight. Koscheck used his wrestling to pin and hold Daley, preventing Daley from mounting any offense. At one point on their feet Daley threw a knee to a downed Koscheck's head, which Koscheck exaggerated to the referee as an illegal strike, pretending his vision had been impared. In replays it showed that Daley's knee whizzed past Koscheck's head and made no contact. Daley was fired and banned from the UFC after the post-fight cheap shot.

    Kubilay acted like a maniac in that fight, eventually getting rear naked choked by the referee. I think he had one more fight after that and his career was over.

    Heath Herring had a largely successful career in Japan up until the punch on Nakao, after which he went 2-3 in the UFC and flamed out of the sport. Nakao took on the nickname "Kiss" and fought sparsely for another five years.

    Gilbert Yvel is a repeat offender. In 1998 he had a bad biting incident against Karimula Barkalaev. In 2001, he was disqualified for repeatedly raking Don Frye's eyes. He struck the referee in the above gif in disagreement with the way the referee was interrupting the clinch work.

    A few more dishonorable mentions:

    Gerard Gordeau, one of the UFC's first combatants, was a hugely successful karate fighter who had difficulty integrating into MMA and grappling. He blinded opponent Yuki Nakai with intentional gouging in 1995.

    Cheick Kongo, also known as Cup Check Kongo, kneed opponent Mirko Cro Cop in the groin at least three times at UFC 75 in 2007 without having any points deducted by the referee. To the dismay of the London crowd, Kongo would take the fight by unanimous decision.

    Takanori Gomi knocked out Luiz Azeredo at Pride Bushino 7 in 2005 with a stunning two-punch combination--then went ape shit on Azeredo's limp body, punching and kicking it until being desperately restrained by the referee and ringside coaches and officials.

    Renato Sobral was infuriated by David Heath's disrespect in the lead up to their fight at UFC 74 in 2007. When he sunk in an anaconda choke on Heath during the fight, he wouldn't let it go, tightly trapping Heath's unconscious body as the referee tried in vain to pull him off. He was subsequently fired and banned by the UFC, though Heath suffered no ill effects from the prolonged blood choke.

    While Rousimar Palhares might not be the best submission artist in the UFC, with the likes of Demian Maia and Roger Gracie about, he is definitely the most feared. He has repeatedly injured opponents in grappling competitions and in MMA. At UFC 111 in 2010 Palhares secured a heel hook on opponent Tomasz Drwal but refused to let go after Drwal tapped-out, wrenching the submission tighter as Drwal screamed. Palhares was suspended for three months following the incident, perhaps only because Drwal did not suffer ligament damage as a result.

    Quinton Jackson and Dan Henderson, longtime friends, both have the ignoble distinction of attacking unconscious opponents to make a point. After knocking out Wanderlei Silva in 2008, "Rampage" Jackson stood over Silva's body and punched him in the head 3-4 times before the referee could intervene. Likewise, in his win over Michael Bisping in 2009, Henderson landed a one-punch knockout that toppled the Brit, then launched into a diving punch that bounced Bisping's unconscious head off the mat one last time in an effort to "shut him up a little".

    Longtime MMA and BJJ practitioner Renzo Gracie, generally regarded as one of the "nice guys" of MMA, kicked Nate Marquardt in the face after his fight with Ricardo Almeida. Almedia had just tapped Marquardt out with a guillotine choke, but when the referee separated the fighters Marquardt threw a punch at a celebrating Almeida. Gracie, cornering Almeida at the time, jumped into the ring and kicked Marquardt in the mouth, sparking a brief brawl in the ring.
