DICE, who creates Battlefield, and another talented developer making the campaign... DICE makes awesome games, and they are working with real Tier-1 soldiers to make this game. Something Infinity Ward never did.

Fallout: New Vegas is hardly going to be an expansion. It's a COMPLETELY different setting, completely different characters, different world, new enemies, etc. If you enjoyed Fallout 3, I think you'd make a mistake by NOT getting Fallout: New Vegas. It's going to be one of the few games, for me, that will give Mass Effect 2 a run for it's money for GOTY.
