The speeding analogy actually backs my point. The tickets don't stop people from speeding. I see it and do it every day. But if I knew I only needed to be busted twice for speeding and I would have my license suspended? Now were talking...
Removing a player from the game until they want to PLAY the game is all good by me. I'm just saying like you did, the fines don't work.
Ignoring speeders wouldn't work either, EVERYONE would speed then because they get somewhere faster.
Ignoring the floppers wouldn't provide them a benefit, but they will continue to do so in the off chance they can fool the ref. Lets remember, a ref calls a flop a foul because he was fooled. Not because he thinks the flop was actually a foul... or he is fooled into thinking so. So how do you ignore what you don't really know, or are fooled into thinking something else?
Must be a punishment.

In NCAA/NFHS "faking being fouled" is a tech. I've yet to see a ref enforce it. Not sure if I want to be that ref but damn... techs would work. When a ref FINALLY does... I'm going to make sure to get the video.

Quick story:
I'm doing a varsity Forest Grove vs Lincoln. After the captain's meeting the little point guard tells me
"We take a lot of charges".
I tell him:
"you'll get every one you earn tonight".
The first 2 block/charge decisions I have make are complete flops (what I expected tbh). I give him eye contact while he's on the ground to let him know I see what happened. I smirk and leave his ass on the floor. A 3rd one (in a row) happens. He looks at me, again while on the ground, and says
"C'mon ref!"
I make eye contact again, smile and say:
"You can't play defense or offense on your butt can you? Get up and stop flopping."
Now, I've made it abundantly clear that I'll leave you on the floor and you will be a detriment to your team while you're there. Afterward we're administering a free throw and the coach says:
"We took 65 charges last season."
I say:
"If your kids keep flopping you won't see one called tonight."
This all happened within the first 4 mins of the game. There were no more flops the rest of the game.
This is the tactic we use and that's why there isn't a rampant flopping problem in NCAA/NFHS.
Not to sound like an ass but your last paragraph is not speaking from the experience of actually changing player behavior. It's a bit of armchair Quarterbacking. The first paragraph I completely agree with.

How I interpret dead ball in my opinion is that the only way the ball comes back into play is when the ref allows it by blowing his whistle and handing the ball to the player... if the players play the ball immediately after anything, it's not a dead ball.
A dead ball happens whenever my whistle is blown. It stops the clock. I don't have to blow my whistle to start play. I do it at key moments like the start of the game or quarter. Or if a huge play happens that causes a good amount of crowd noise. I always whistle the play dead though (fouls, infractions, timeouts).
